Appointments
Appointments will ordinarily be 45-55 minutes in duration and occur once or twice per week at a time we agree on, although some sessions may be more or less frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, please provide at least a 24-hour notice.
Insurance
At Embark Mental Health, it is important for us to understand your resources for payment of your appointment either through insurance or private pay. Most insurance companies cover some of your mental health treatment. With your permission, our billing service will help in the extent possible in filing claims and ascertaining information about your coverage, but you are responsible for knowing your coverage, understanding authorization requirements, and for notifying Embark Mental Health if/when your coverage changes.

Before starting therapy services, these are some helpful questions to ask your insurance provider:
Do I need a referral from a Primary Care Physician for services to be covered?
What mental health benefits are included in my insurance plan?
Does my specific insurance plan have a limit on how many sessions I have can per calendar year? If so, what is the limit?
What is my deductible or co-payment? Have I met my deductible?

Payment
You will be sent a monthly bill that reflects your charges, what you have paid, and what your insurance company has paid.
Ultimately, you, and not your insurance company, are responsible for the full payment of your bill.

Late Fees:
Late fees are as scheduled and will begin after 90 days:
- Outstanding balances up to $250 will have a late fee of $25.
- Outstanding balances From to $251-$500 will have a late fee of $50.
- Outstanding balances From to $501-$750 will have a late fee of $75.
- The pattern of late fees continues incrementally and will be applied at the end of each billing cycle.
